FSD Participates in Culture of Health Learning Collaborative
The Robert Wood Johnson Foundation and the Health Research & Educational Trust (HRET) of the American Hospital Association have embarked on an exciting venture to foster a culture of health amongst hospitals and community organizations across America.
With this goal, they have created the “Culture of Health Learning Collaborative,” a select group of ten hospitals across the country who partner with community organizations. The partnership between Sharp HealthCare’s Care Transition Program (CTI) at Sharp Grossmont Hospital, Feeding San Diego and 2-1-1 San Diego was chosen to participate in the collaborative as an example to help develop best practices. In coordination with care provided by Sharp Care Transition coaches, FSD currently provides food boxes to patients who are identified as food insecure during the hospital discharge process. Additionally, individuals are referred to the FSD CalFresh Outreach team to be screened for eligibility and directed to other FASD food distributions and 2-1-1 San Diego for additional services to meet their needs.
Pictured above, representatives from the HRET visited San Diego on March 24 and 25 to learn more about the program and visit with FSD and 2-1-1 San Diego. Over the next two years, HRET will work to develop best practices for hospitals across the country using these partnerships as examples.
